Now I know that mbaacctool is used for keyboard input config, but not all of the commands available to a joystick are configurable, notably A+B. How might I configure this to a keyboard?

AFAIK there's no way to config the A+B macro through mbaacctool. However, you can use a program such as AutoHotkey to create custom macros.
